Characteristics of Atopic Dermatitis Associated with Severe Ocular Complications

Background: The patients with atopic dermatitis(AD) have predisposition to ocular complications. Especially, cataract and retinal detachment(RD) are the most serious complications that may cause visual disturbance. But they often occur without specific symptoms and it delays appropriate treatment and causes permanent sequalae.
Objective: The aim of the present study was to investigate the characteristics of AD associated with severe ocular complications in Korea. Materials and Methods: We reviewed medical records of 29 AD patients(24 males, 5 females) who had cataract or RD. Clinical data were also collected through questionnaires answered by the AD patients and, if necessary, through telephone interview with a dermatologist.
Results: Mean age of the patients was 26.7 years. The onset of AD during childhood was most common(17 patients, 58.6%). Cataracts were seen in 29 patients(24 males, 5 females, 44 eyes) and RD¡¯s in 16(13 males, 3 females, 22 eyes). All the patients with severe ocular complications had facial dermatitis and duration of AD more than 10 years. And other ocular involvements including uveitis(7 patients), glaucoma(4 patients), keratoconjunctivitis(3 patients) were also seen.
Conclusions: We found that the AD patients do not have knowledge about the ocular complications associated with AD and visited ophthalmologists after onset of evident visual disturbance. Therefore, we suggest that for an early diagnosis and timely treatment of severe ocular complications, it is important to perform a regular ophthalmologic examination in AD patients with facial skin lesions who are more than 10 years old, especially when they are male or have long-standing history of AD.
